At Team 24 Care, we offer a single, comprehensive referral form that covers all of our services. You can find the form by scrolling down. We kindly request that Care Coordinators include a current CPA risk assessment and care plan when submitting the referral.

Our referrals mainly come from statutory secondary mental health services, including community mental health teams and inpatient facilities. However, we also welcome referrals from primary care providers, as well as direct submissions from individuals and carers.
