At Team 24 Care, we offer a comprehensive referral form that covers all of our services. You can find the form by scrolling down this page. We kindly ask that Care Coordinators include a current CPA risk assessment and care plan when submitting a referral.

Our referrals come from statutory secondary mental health services, community mental health teams, and inpatient facilities. Additionally, we accept referrals from primary care providers, local authorities, as well as direct submissions from individuals and their carers.
